At one point he had a foster mother named Florence, a foster sister named Melissa, and an unnamed foster brother with whom he played mousetrap. While John's grandmother discussed adoption with a nurse, Richard Alpert smiled at baby John from an adjoining room. Emily said she couldn't do it, possibly referring to having to raise a child. John's mother, Emily, ran out of the hospital ward upon learning that John was to be let out of his incubator for the first time and that she could hold him. Three months premature, John survived numerous illnesses his nurses called him a miracle.

Locke spent the next four years of his life paralyzed from the waist down and looking for his purpose in life. Locke grew up in foster care, but later in life, he met his father, who conned him into giving him his kidney under false pretenses of love, ruined his relationship with his girlfriend, and finally pushed him out of a window, breaking his back and confining him to a wheelchair. In service to this cause, Locke became a "Man of Faith", sparking an increasingly tense rivalry with Jack who was in contrast a "Man of Science". He, Rose, and Walt were the only survivors who wanted to stay on the Island, believing his special destiny was to protect the miraculous place. A disabled wheelchair user at the time of the flight, Locke miraculously found himself able to walk once he arrived on the Island. John Locke was a survivor of Oceanic Flight 815.
